Download Pins controls automatic downloads of pinned items. It does not determine whether an eligible item can be pinned or whether the pin should be visible at the top of your Library.

If your complaint is “the pin is there, but the music is not available offline,” open Settings > Apps > Music and turn on Download Pins. If the complaint is “there is no pin” or “the pin is missing on another device,” focus on Library eligibility, subscription/account status, and Sync Library instead.
